The services we commission for adults living in Camden is focused on a range of priorities, including:
- Achieving early diagnosis and better outcomes for people with long term conditions, and value for money for the services we commission for these patients.
- Collaborating with Camden Council to plan and commission many community health services for adults.
- Working hard to increase earlier diagnosis of cancer, through educating Camden clinicians, raising awareness among groups most at risk, and working in partnership to plan cancer services with colleagues across north central London.
- Improving outcomes and increase earlier diagnosis of people living with dementia.
The Camden Local Care Strategy aims to increase adult’s independence at home, reducing unplanned hospital admissions and improving experience of health and care services. An important part of this will be looking at how best we can support people in their own home, through joining up community nursing, therapists and GP services.
